When patients visit their primary care physician with specific concerns, the doctor evaluates their condition. If specialized care or advanced diagnostics are necessary, the doctor discusses the need for a referral. Referrals follow a process that typically involves selecting an appropriate specialist or clinic based on the patient’s condition, insurance, and preferences. This process usually takes time, and referrals generally do not occur on the same day the patient sees the primary care physician.
Once the referral decision is made, the doctor prepares a referral document that includes the patient’s medical history and reason for referral. This referral is then sent to the receiving clinic or provided to the patient. The patient or the referring clinic schedules the appointment with the specialist, and the patient receives confirmation details for the visit.

Becoming a new patient is simple and straightforward. Start by calling the clinic to schedule an appointment, and be sure to confirm that they accept your insurance. Prior to your appointment, fill out new patient forms, which you can find here. These forms help the clinic understand your medical history and insurance details, ensuring you receive the best care possible from your first visit.

Opt-Out Instructions
You can opt out of SMS messages by texting STOP to the respective shortcode. You can opt out of artificial or prerecorded voice calls by following the menu in the call. Your opt-out request will generate one final message confirming that you have been unsubscribed. Once your decision to opt out is confirmed you will no longer receive the type of message you opted out from. If you want to join again, sign up using Follow My Health.
