Things to Know for Your First Visit
To ensure you have enough time to fill out your medical history and insurance forms, the KIMA team asks that you please arrive 15 minutes prior to your scheduled appointment time. The following documents are also required on your first visit:
- Any referral or authorization
- Photo identification
- Proof of eligibility or insurance card

Accommodating Your Needs With Scheduling
The KIMA staff wants to work with you to find a reasonable appointment time that fits into your schedule, and in order to accommodate all of our patients, appointments are required to be seen.
In order to ensure timely service, you must cancel your appointment 24 hours in advance of the scheduled time.
Based on the type of patients KIMA physicians treat, emergencies can happen, and those patients may be seen before patients with appointments, depending on the severity of the case.
All KIMA physicians also work on-call for the treatment of acute illnesses, which may result in your physician being called to the hospital without notice. Should this happen, the KIMA staff will notify you as soon as possible and will work with you to make alternate arrangements.
Billing and Insurance
As an active participant in several managed care plans, KIMA requires payment to be made at the time of service. Only the co-payment and any balances not covered by your insurance provider are due at the time of your office visit.
KIMA accepts several types of insurance and will file a claim with your insurance company for the balance of your bill, if necessary. To clarify if your insurance is accepted or if the doctor you need to see is affiliated by your insurance plan, please contact our office before your visit.
